Documentation of Adult Attention-Deficit / Hyperactivity Disorder

The University of Guelph requires that students diagnosed with Adult ADHD provide current, comprehensive documentation by a qualified professional to support their requests for accommodation, or to support application for disability funding.

This documentation must include:

  • the current diagnosis of adult ADHD,
  • evidence that the disorder is disabling and interferes significantly with academic functioning,
  • description of the functional limitations in a university setting, and
  • disclosure of any co-existing conditions  E.g. specific learning disability, anxiety, depression, mood disorders or addictions that might be interfering with academic functioning.

This document will be a current* psycho-educational assessment completed by a registered psychologist or psychological associate.   The assessment must follow the Guidelines for Documentation of Attention-Deficit/Hyperactivity Disorder established by the U of G.  The assessment must investigate both learning disabilities and ADHD.

* Generally, this refers to an assessment completed in the last 3 years, or a diagnosis made at 18 years of age or older.
