The Health and Performance Centre (HPC) is committed to maintaining the accuracy, confidentiality and security of your personal health information. The HPC and Student Health Services at the University of Guelph offer a multidisciplinary approach to services and information and share a Clinic Information System for registration, scheduling, billing and clinical record management. At all times, the staff, practitioners and volunteers of the HPC will conduct themselves in a professional manner ensuring the highest quality of care, service and confidentiality. They are trained in the appropriate use and protection of your information.
Important Information:
- - We take steps to protect your personal health information from theft, loss and unauthorized access, copying, modification, use, disclosure and disposal.
- - We conduct audits and complete investigations to monitor and manage our privacy compliance.
- - We take steps to ensure that everyone who performs services for us protects your privacy and only uses your personal health information for the purposes you have consented.
Collection of Personal Health Information
We collect personal health information about you directly from you or from the person acting on your behalf. The personal health information that we collect may include: your name, date of birth, address, email address, health history, records of your visits to Health and Performance Centre, and the care that you received during those visits. Occasionally, we collect personal health information about you from other sources (e.g. your motor vehicle insurer) if we have obtained consent from you to do so or if the law permits.
We use and disclose your personal health information to:
- - Treat and care for you;
- - Allow us to maintain communication and to book and confirm appointments;
- - Communicate with other treating health-care providers, including specialists;
- - Receive payment for your treatment and care (from OHIP, UHIP, WSIB, your private insurer, motor vehicle insurer or others, or Provincial Health Plans);
- - Invoice for services, collect unpaid accounts and process payments by cash, credit card, cheque or debit;
- - Compile statistics and plan, administer and manage our internal operations;
- - Conduct risk management and quality improvement activities;
- - Teach and research in accordance with applicable legislation;
- - Retain and destroy records in accordance with the law;
- - Comply with legal and regulatory requirements;
- - To allow governing bodies and University auditors to conduct audits when requested for regulatory and/or monitoring purposes;
- - To prepare materials for the Health Professions Appeal and Review Board (HPARB) if necessary; and
- - Fulfill other purposes permitted or required by law.
Consent:
Your request for care and service at the Health and Performance Centre implies consent for the collection, use and disclosure of your personal information for purposes related to your care.
Your Choices:
You may access and correct your personal health records, or withdraw consent for some of the above uses and disclosures by contacting us (subject to legal exceptions).
