Incident/Injury Reporting During the Holidays

If an incident occurs during the holidays that requires:

• External medical attention
• Hospital visit or
• Lost time from work

Please fax the incident report form to Occupational Health and Wellness at 519-780-1796.

In the event of a crital injury, please refer to the following procedures for main campus and offsite colleges (i.e. Kemptville).
