button link Safety Orientation Checklist

  • A record of safety orientation given to a new employee or incoming student upon hiring.

  • A record of safety training given to volunteers, work study students, teaching assistants, etc. to promote due diligence when individuals are engaged to provide service to the University of Guelph, and in particular to our undergraduate students in the teaching laboratories.

  • The supervisor is responsible for ensuring this form is completed and copies are filed with the department

button link Incident/Injury Report

  • Must be filled out within 24 hours of incident.
  • Fax to 519-780-1796 and the fax will be received by both OHS and EHS Purpose of the Incident Report Form:

    The incident report form has been designed to ensure compliance with Workplace Safety and Insurance Board and Ministry of Labour regulations, which require reporting an occupational injury or disease within 24 hours of the occurrence. The information requested on this form will be used by Occupational Health Services (OHS) for the completion of the WSIB Form 7 (The Employers Report of Injury or Disease) and by the Environmental Health and Safety Department (EHS) to provide information to the Ministry of Labour should it be requested and/or to ensure that the area supervisor is aware of, and has followed-up on, the injury and/or property damage that has occurred.

  • After faxing to OHS/EHS, distribute Copies to: 1.Dept Head 2.Union/Bargaining Group/Staff Association Available online (instructions on second page of form)

button link Field Research Safety Form

  • Refer to University of Guelph Safety Policy 851.06.04 for additional details This form must be sent to the Chair of the Department of Integrative Biology for approval. Signature of Chair of IB required.
  • Principal Investigator keeps a copy
  • Department keeps a copy
  • Department sends completed form, minus Appendices, to Risk & Insurance Manager (5th Fl. UC) as per 851.06.04

button link Field Trip Safety Form

Must be filled out to go on trip. To be submitted to the IB Department Chair for approval prior to entering the field or going on the excursion. Refer to University of Guelph Safety Policy 851.06.23 for additional details.

Principal Investigator keeps a copy. Department keeps a copy. Department sends completed form, minus Appendices, to Risk & Insurance Manager (5th Fl. UC) as per 851.06.23

button link Lab Inspection Form

The responsibility of IB Safety Committee Members is to inspect all labs associated with the department of Integrative Biology on a yearly basis. A record of each inspection is to be sent to the Area Supervisor; IB Department Chair; CBS Dean & Environmental Health & Safety Department.
