Lepiota cristata            Stinking Parasol

In my experience this is the most common of the woodland Lepiota species in the Great Lakes region and probably also further east.   It is a small mushroom seldom exceeding 4 cm across and has an unpleasant smell..  The pale red-brown scales are typical and it tends to fruit in small clusters.  The ring is delicate, movable and disappears in age or if handled.  It often fruits in grass at the edge of trails.   It is reported as poisonous so watch out for the kids!
