Supplementary Documentation (International Students)
After you have applied, you must complete and submit supplementary documentation for specific programs. See below for required and optional supplementary documentation forms.
Required Supplementary Forms
The Turfgrass Background Information Form is required for all applicants to the Associate Diploma in Turfgrass Management program. Forms and other required documents received after February 16th will be considered, space permitting.

The BLA Background Information Form (BIF) is required for all applicants to the Bachelor of Landscape Architecture Program.

Deadline: March 1, 2024 (High School Applicants); May 1, 2024 (Transfer Applicants)
The DVM Background Information Form is required for all applicants to the Doctor of Veterinary Medicine program.

Applied Science Personal Letter: In order to assist the Bachelor of Applied Science Admissions Committee in selecting the most qualified applicants, you are encouraged to submit a letter about yourself. The committee is most interested in your background and motivation as it pertains to the study of applied science.
Deadline: June 1, 2024
Transfer applicants who have factors beyond their control that have negatively affected their academic performance, such as illness or personal circumstances, are encouraged to submit a support letter.

Optional Supplementary Documentation Forms
We recognize that non-academic factors play an important role in students' lives. The Student Profile Form (SPF) is your opportunity to tell the Admission Committee about activities that you have been involved in, and that have helped you develop leadership and citizenship qualities. We will also consider factors beyond your control that have negatively affected your academic performance, such as illness or personal problems. If your admission average in May does not meet the cut-off for the program to which you have applied, your SPF will be considered if your average falls within 5% of the determined cut-off.

Upon receipt of your application for admission to U of G, you may have been asked to complete the Education Declaration Form. You will know if you are required to complete this form by reviewing either communication received by Admission Services which specifies what supporting documentation you are required to submit or if you see "Education Declaration Form" as a required document on your Admission Portal. If so, please complete the above PDF and submit it to applicant@uoguelph.ca
