In Person:
Complete the "Request for Transcript" form and submit to Academic Records, University Centre Level 3, with the appropriate payment (cheque or money order accepted).
By Mail:
Mail a letter of authorization that includes your full name, University of Guelph I.D. number (or S.I.N. number AND date of birth), the complete and current mailing address(es) you would like the transcript(s) mailed to, a complete and current mailing address for yourself to which a receipt may be sent, your signature, and the appropriate payment in the form of a cheque or money order to:

By Facsimile:
