CUPE 1334 Mid-Probationary Review

All employees covered by the CUPE Local 1334 Collective Agreement [1] are provided with a mid-probationary period progress report. This report is to be completed at approximately three (3) months into their six (6)-month probationary period. 

The relevant Collective Agreement language in Article 12.02 reads:

“A new employee shall be on probation until completion of six (6) months continuous employment. Probationary employees will be given a written progress report upon completion of three (3) months’ service. Both the employee and the supervisor will be notified that a review is required."

Please complete the CUPE 1334 Mid-Probationary Review Form [2] and engage in discussion with the employee, returning the completed form to your Service Associate [3]. Comprehensive instructions are also provided on the form.
