Ontario Medal for Good Citizenship

Description

This medal program honours individuals for their exceptional long-term efforts and outstanding contributions to the well-being of their communities throughout the province.

Eligibility

The person you nominate (the nominee) must be a resident of Ontario.

You cannot nominate:

  • yourself
  • someone who has passed away
  • an elected federal, provincial or municipal representative currently in office
  • a public appointee if their achievements relate to their current appointment
  • someone for acts of bravery

Prize

The winner is presented with a medal.

If College-level review is required, your College will communicate its earlier internal deadlines.

External Deadline

Please submit the nomination directly to the Ontario's Honours and Awards Portal website by the external deadline. Please also submit a copy of the full nomination and OR-5 to research.services@uoguelph.ca by the external deadline.

How to Apply

To submit a nomination, you will need to provide:

  • The nominee’s first name, last name, email address and phone number.
  • The nominator’s first name, last name, email address and phone number.
  • A detailed description of the nominee’s achievements.
  • At least two signed testimonial letters.
  • The testimonial writers’ first name, last name, email address and phone number.

Applications must be submitted online using the application form provided on the Ontario's Honours and Awards Portal website by the external deadline. Please also submit an OR-5 and a completed application to research.services@uoguelph.ca by the external deadline.
