Updated Human Participant Face to Face Research COVID-19 Consent Appendix

A revised version of the COVID-19 Consent Appendix has been released. This Appendix provides human research participants with pertinent safety information and instructions related to their in-person research visits and procedures. This new version changes the way a participant’s vaccine status is reported. Instead of collecting vaccine status in the Appendix itself, participants will now use the University of Guelph Daily Screening form to attest to their vaccination or exemption status.

Researchers who already have Research Ethics Board approval in place with an earlier version of the COVID-19 Consent Appendix may continue with their studies as they have been, but are reminded that care must be taken to store consent documents according to the guidance provided for S3 data in the University of Guelph Data Storage Guidelines.
