This form is to be completed by University of Guelph faculty who collaborate on an NSERC CREATE Grant and supervise graduate students who are funded through the NSERC CREATE grant.

The Tri-Council Top-Up is valued at $5,000 per year and is credited to the student's account in three equal installments annually.  The student is eligible to receive the Tri-Council Top-Up for the semesters they are being funded through the NSERC CREATE program.  Students must remain registered full-time at the university to be eligible for the Top-Up.
