College of Biological Science Student Questionnaire Feedback

Below is an example of the CBS Student Questionnaire Feedback Form distributed to students at the end of each semester by the Department of Human Health and Nutritional Sciences

The College of Biological Science provides students with this opportunity to evaluate the quality of instruction that they have received. This feedback is seriously considered during the evaluation of your Instructor's career. Please answer these questions thoughtfully. In addition to your responses below, comments on the instructor, method of instruction or the course content can be included directly on the form or in a separate letter to the Chair of the Department. Your Instructor may use anonymous comments to improve the course. If you wish to have your comments used in the professional evaluation of your Instructor, you must sign this form. Your identity will be made available to the Instructor but only after the final marks have been submitted.
