Do I need to include the University of Guelph logo or my departmental or College letterhead on my consent form? If researchers from more than one institution are involved in the research, should I include their logos as well?

Yes. The logos on a consent form should reflect the range of institutions which are involved and oversee the project. More than one logo can appear, for instance, in the case of a cross-appointment where lines of reporting exist to more than one institution/agency/organization.

You do not need to have the logo or letterhead in place for ethics review. Simply indicate at the top of the document that it will be printed on letterhead or have a logo added prior to use.
